1. Please read, sign and return the Rental Agreement form below.
2. A COI will be requested from your production made out to additionally insured for Apollo Lighting & Grip, please follow the COI example below.
3. Review the current equipment inventory and add any ala carte, expendables or additional items you may need for your production.
If paying by credit card, please fill out the Credit Card Authorization Form.
